Clinical Research for Diagnostic and Surgical Approaches of Obturator Hernia

Abstract
Purpose: Obturator foramen hernia is a rare type of hernia which is around 0.05¡­1.4% of prevalence. In this study of a patient undergoing obturator hernia operation, we hope to find out the useful methods of diagnosis and operative treatments.

Methods: This study included a total of 7 patients who received treatment from a single hospital for obturator hernia from April 2007 to March 2012. Analysis of patients were made through retrospective approaches which involve medical records and telephone surveys.

Results: All 7 patients were females over 80 year old, and were multiparous with an average parity of 4.3. All 7 patients were also inpatients and visited the ER with symptoms of bowel obstructions such as abdominal pains or vomiting, and symptoms of abdominal discomforts such as nausea. The diagnosis of all patients were confirmed by an abdominal computed tomography (CT). All 7 patients had small bowel incarcerations, 3 had co-existing small bowel strangulations and underwent small bowel resections and the obturator underwent primary sutures. After reduction of the small bowel in the 3 patients, they were approached from the preperitoneal space around the area of skin incision. A cone-shaped artificial mesh was placed on top of the preperitoneal space in order to cover the defect. After they were discharged, 6 patients were followed up for 3 months and no complications or recurrences were found.

Conclusion: In this study, the abdominal CT of patients was used as a useful tool for the diagnosis. The cone-shaped mesh has been used safely without recurrences and complications.
